A lengthy final report has been released detailing the official cause of the Givaudan plant explosion.The blast happened in November 2024. Two workers were killed and three were seriously injured. The plant itself, which stood in the middle of the Clifton neighborhood surrounded by homes, was destroyed.“This tragic incident was a catastrophe waiting to happen,” CSB Chairperson Steve Owens said.On Wednesday, the U.S. Chemical Safety and Hazard Investigation Board released a 96-page report.Investigators concluded that Givaudan didn’t fully understand the chemical hazards involved in its processes.They also said the company had weaknesses in safety planning and training.The report says that a 2,500-gallon reactor exploded because a sugar ingredient used for the food coloring overheated, broke down too fast and created dangerous pressure.”The resulting rapid pressure increase overwhelmed the reactor's emergency pressure relief system, causing the reactor to rupture violently,” the board said in a news release.A vent valve appears to have stayed shut when it should have opened, which made the overheating much worse. The report says the emergency pressure-relief system worked, but it was too small to handle the massive reaction.
